Standardized Assessment and Person-Centred Care Planning
An Operator must ensure that care planning begins upon the Client’s commencement of Home and Community Care, or upon admission to a Type A Continuing Care Home or Type B Continuing Care Home and that the Care Plan:
- reflects the findings of the assessment in 1.1;
- is kept up to date and relevant to the Client's Health Status; and
- is revised by a Regulated Health Care Provider based on any reassessments.
