Standardized Assessment and Person-Centred Care Planning
An Operator must ensure a Client or the Client's legal representative, if applicable, have the opportunity to:
- participate in the development and review of the Client's Care Plan, including the determination of Health Care needs and service options;
- invite individuals of their choosing to participate in the development and review of the Care Plan; and
- access the Client's Care Plan upon request.
