Standardized Assessment and Person-Centred Care Planning
An Operator must ensure that all Care Plan reviews address whether:
- the Care Plan addresses the Unmet Health Care Needs of the Client;
- the Client’s Health Care needs and goals are being met;
- the interventions that have been implemented related to the Client’s Health Care needs and goals have been effective; and
- any revisions are required.
