Standardized Assessment and Person-Centred Care Planning
An Operator must ensure that the Care Plan addresses:
- a Client's physical, mental, emotional, social, intellectual and spiritual Health Care needs and corresponding goals;
- a description of the necessary interventions related to the assessment in 1.1 and which Interdisciplinary Team member is responsible for providing those interventions; and
- where a Client has a legal representative:
- identification of the Client's legal representative;
- identification of the source of their legal authority; and
- contact information for the legal representative.
