Restraint Management and Secure Spaces

When a Restraint is used, an Operator must ensure the following is documented in a Client’s chart and Care Plan:

  1. the behaviour that put the Client or others at risk of harm;
  2. the supportive interventions that have been considered and trialed;
  3. indications for the initial use of the Restraint;
  4. a Physician’s order or Nurse Practitioner’s order, within 72 hours of initiation of the Restraint, authorizing the use of the Restraint;
  5. the method and frequency for monitoring the Client when the Restraint is in use; and
  6. assessment of the Client while the Restraint is being used and review of the ongoing need for the Restraint.
