Quality Improvement Reporting

An Operator must establish, implement, and maintain, documented policies and procedures for the documentation, tracking, and trending of:

  1. any incident that could pose an adverse risk to a Client; and
  2. any near miss that could have resulted in negative consequences for a Client but did not because of chance or timely intervention.
