Quality Improvement Reporting

A reportable incident is an unexpected or normally avoidable outcome that negatively affects a Client’s health or quality of life and occurs in the course of Health Care or has the potential to alter the Client’s Health Status. Reportable incidents include:

  1. death or serious harm to a Client caused by:
    1. error or omission in the provision of Health Care;
    2. error or omission in the provision of accommodation services;
    3. equipment malfunction or error in operation;
    4. accommodation grounds or equipment in disrepair or unsafe; or
    5. assault/aggression;
  2. Client being unaccounted for;
  3. unplanned activation of a contingency plan caused by:
    1. disruption of utilities;
    2. evacuation;
    3. Staff disruption;
    4. severe weather; or
    5. loss of essential equipment.
  4. extensive damage to the accommodation caused by:
    1. fire or flood;
    2. disaster; or
    3. building or equipment failure.
