Quality Improvement Reporting
A reportable incident is an unexpected or normally avoidable outcome that negatively affects a Client’s health or quality of life and occurs in the course of Health Care or has the potential to alter the Client’s Health Status. Reportable incidents include:
- death or serious harm to a Client caused by:
- error or omission in the provision of Health Care;
- error or omission in the provision of accommodation services;
- equipment malfunction or error in operation;
- accommodation grounds or equipment in disrepair or unsafe; or
- assault/aggression;
- Client being unaccounted for;
- unplanned activation of a contingency plan caused by:
- disruption of utilities;
- evacuation;
- Staff disruption;
- severe weather; or
- loss of essential equipment.
- extensive damage to the accommodation caused by:
- fire or flood;
- disaster; or
- building or equipment failure.
