Coordinated Access
Coordinated Access provides a roadmap for the client journey through the continuing care system, beginning with inquiry for information, intake and screening; assessment; service needs determination, identification of individual service options, service recommendation and referral, service delivery, monitoring/reassessment, transition, discharge and waitlist management.
This work sets the stage for a province-wide, person-centered approach to integrated service delivery that provides Albertans with reasonable, timely and appropriate access to publicly funded health care based on availability and determination of unmet need.
These resources are applicable to all Care Streams unless stated in the title of the document.
