Collaborative Care is grounded in three principles: patient/resident and family centred care, team effectiveness and functioning, and safe, high quality care delivery. The CoACT Collaborative Care Framework details the practices, processes, enablers and partnerships integral to advancing Collaborative Care and achieving optimal outcomes.
The Collaborative Care Framework for Continuing Care is included in the Continuing Care Quality Plan and aligned with Health Service Standards, Accommodation Standards, AHS Leadership Systems, Accreditation, and the Resident & Family Council Act.
